Young, Fit, And Diagnosed: Why Cancer Is No Longer Just A Disease Of Old Age
‘In my 25 years as a surgical oncologist, I have witnessed many shifts in cancer patterns," says Dr Trilok Pratap Singh Bhandari, Senior Consultant, Surgical Oncology, Apollo Cancer Centre, Hyderabad. “But none have been as concerning as the steady rise of cancer among young adults."
Early in his career, Dr Bhandari explains, the majority of his patients were in their 50s or 60s. “Today, it is increasingly common for patients in their 30s or even younger to walk into my clinic," he says. “They often appear fit, active, and healthy, completely unprepared for a cancer diagnosis."
Breast cancer in younger women
Breast cancer is one of the most striking examples of this trend. “Over the years, I have seen a clear increase in the number of young women diagnosed with breast cancer," notes Dr Bhandari. Many of these patients are working professionals and mothers of young children, often perceived as the healthiest individuals in their families.
“In Indian clinical practice, nearly 10 to 20 per cent of breast cancer patients are now premenopausal," he explains, adding that this proportion continues to rise. “What is especially worrying is that younger women often present with more advanced disease. Cancer is usually the last thing they suspect, and routine screening is still largely associated with older age."
Colorectal cancer no longer rare in the young
A similar shift is being seen in colorectal cancer. “When I began practising, colorectal cancer in patients under 40 was rare," says Dr Bhandari. “Today, I regularly see young adults presenting with rectal bleeding, unexplained weight loss, or persistent changes in bowel habits."
He points out that many patients ignore symptoms for months or even years attributing them to haemorrhoids, stress, or dietary issues. “By the time they seek specialist care, the disease is often already at an advanced stage," he adds.
Lifestyle changes at the core
From Dr Bhandari's day-to-day clinical experience, lifestyle changes play a significant role in this alarming trend. “Rapid urbanisation in India has transformed the way we eat, work, and live," he explains.
“Traditional diets rich in fibre, pulses, vegetables, and whole grains are being replaced by processed foods, refined carbohydrates, sugary beverages, and fast food," says Dr Bhandari. At the same time, physical activity has declined, working hours have increased, sleep patterns are disrupted, and obesity is rising, even among younger populations. “All of these are well-established risk factors for cancers such as breast and colorectal cancer."
Changing reproductive and social patterns
Among young women, additional factors appear to be contributing to earlier onset breast cancer. “Delayed childbearing, reduced breastfeeding, chronic stress, and sedentary lifestyles are becoming more common," explains Dr Bhandari, “and these changes likely influence the rising incidence of breast cancer in younger age groups."
Similarly, he notes that low-fibre diets, higher intake of processed and red meats, and lack of physical activity are closely linked to the increasing burden of colorectal cancer among young adults.
Awareness remains the biggest gap
While genetic predisposition accounts for a small proportion of early-onset cancers, Dr Bhandari emphasises that lifestyle and environmental factors are driving most of this shift. “What concerns me the most is not just the rising numbers," he says, “but the lack of awareness among patients and, at times, even healthcare providers that cancer can affect young people."

“Cancer is no longer just a disease of old age," Dr Bhandari concludes. “In today's India, prevention, early detection, and lifestyle modification must begin early. Being young and physically fit no longer offers immunity from cancer."
