Many people believe glaucoma affects only the elderly, but it can impact young adults as well, often silently stealing vision. In India, glaucoma affects nearly 12 million people overall, with approximately 1.2 million cases occurring in individuals under the age of 40. This includes rare paediatric forms such as Juvenile Open-Angle Glaucoma (JOAG).
Dr Sneha Kankaria, Glaucoma & Cataract Specialist, Ayush Eye Clinic, a unit of Dr Agarwal's Eye Hospital, Chembur, shares key insights on the condition.
Key Risk Factors
Family history tops the list, particularly genetic mutations that lead to poor fluid drainage within the eye. Other risk factors include eye trauma, prolonged steroid use, diabetes, high blood pressure, and nearsightedness. In India, studies indicate that primary congenital glaucoma (PCG) accounts for nearly 37% of childhood glaucoma cases and often presents earlier in Asian populations.
Symptoms to Watch For
Young adults may experience blurred vision, light sensitivity, worsening nearsightedness, halos around lights, eye pain, headaches, or loss of side vision. Early symptoms often mimic eye strain or fatigue, so they should not be ignored.
Diagnostic Tools
Comprehensive eye examinations are essential and include measuring intraocular pressure (IOP), gonioscopy to examine the eye's drainage angle, visual field testing, and structural analysis of the optic nerve using Optical Coherence Tomography (OCT). These tests help detect damage before noticeable vision loss occurs.
Latest Treatment Options
Modern eye drops help lower IOP with fewer side effects than earlier formulations. Laser treatments are gaining popularity and are considered first-line therapy in certain types of glaucoma. Minimally Invasive Glaucoma Surgery (MIGS) offers faster recovery using tiny implantable devices. For advanced cases, surgeries such as trabeculectomy remain the gold standard. In secondary and complex glaucomas, implantation of drainage valves may also be recommended.
Living with Glaucoma
Adhering to daily eye drop regimens, protecting the eyes from injury, maintaining a healthy diet, exercising regularly, and avoiding smoking are essential for disease management. Regular follow-ups help track progression and treatment effectiveness.
