Two Plates, Same Food, Different Waistlines: Blame Biology, Not Habits
Two people sit down to eat the same meal, same food, same portion size. Yet weeks later, one gains weight while the other doesn’t. For decades, this paradox has been explained away with a convenient narrative: discipline, habits, or willpower. Science now tells a very different story. The real difference lies in biology.
When a metabolically healthy individual eats, the body responds with remarkable precision. The gut releases a cascade of hormones, GLP-1, peptide YY, and cholecystokinin, that signal satiety to the brain’s appetite centre, the hypothalamus. Hunger switches off. Ghrelin, the hormone that drives appetite, is suppressed. The system resets smoothly after the meal.
As Dr Sukrit Singh Sethi, Director and Senior Consultant in Gastroenterology, Narayana Hospital, explains, “In individuals living with obesity, this gut–brain communication system does not function normally. The GLP-1 response is blunted. Ghrelin suppression is incomplete. The hypothalamus never receives a clear signal that enough food has been consumed. As a result, continued eating is not a failure of self-control, it is the outcome of faulty neuroendocrine signalling. The body is, quite literally, sending the wrong instructions."
Compounding this is a phenomenon known as adaptive thermogenesis. When individuals with obesity reduce calorie intake, their bodies respond defensively by lowering the basal metabolic rate far more than expected. This is not laziness or lack of effort; it is a deeply ingrained survival mechanism developed over thousands of years to protect against starvation. In today’s food-rich environment, this same mechanism actively resists weight loss, making sustained results extremely difficult through diet alone.
Genetics further stack the odds. More than 900 polygenic variants have been linked to obesity risk. These genes influence how fat cells are formed, how the brain processes food reward, how sensitive insulin receptors are, and how efficiently brown fat burns energy. Studies suggest that 40 to 70 percent of obesity risk is heritable. In very real terms, many individuals are battling their own genome.
Then there is the gut microbiome. Individuals with obesity often have a Firmicutes-dominant microbial profile that extracts more calories from the same food compared to other microbiome compositions. This dysbiosis also promotes low-grade inflammation and worsens insulin resistance, particularly in the liver. Once again, identical plates of food are metabolised in fundamentally different ways.
Taken together, obesity is not a behavioural flaw. It is a chronic, multilayered biological disease involving hormonal dysregulation, genetics, metabolism, and the microbiome.
“This is why modern treatment must also be biological. Therapies that correct the gut–brain satiety axis or alter gastric physiology are not shortcuts, they are targeted interventions addressing underlying disease mechanisms. Continuing to moralise obesity ignores decades of scientific progress and delays effective care," adds Dr Sethi.
The evidence is clear. “Obesity deserves the same clinical rigour, empathy, and seriousness that we apply to any other chronic medical condition. It is time we stop blaming habits, and start treating biology," signs off Dr Sethi.
