A new study from Keck Medicine of USC has uncovered which risk factors are most dangerous for people with a very common liver condition—Metabolic Dysfunction–Associated Steatotic Liver Disease (MASLD), formerly known as non-alcoholic fatty liver disease (NAFLD). The findings challenge long-held assumptions about what truly raises the risk of death for patients.
What Is MASLD?
MASLD occurs when excess fat builds up in the liver, impairing its function over time. While it may sound harmless at first, the condition can progress to inflammation, scarring, or even liver failure.
What makes MASLD especially tricky is that it often co-exists with other metabolic conditions such as obesity, diabetes, hypertension, and cholesterol issues. Not all of these factors carry equal weight, however, and the new study sheds light on which ones matter most for survival.
High Blood Pressure: The Silent Killer
The most surprising finding? High blood pressure (hypertension) turned out to be the deadliest partner to MASLD. Patients with both MASLD and hypertension had about a 40% higher risk of death compared to those without high blood pressure.

The second big risk factor is diabetes or prediabetes. The study found that MASLD patients with impaired glucose regulation had about a 25% higher risk of death.
Elevated blood sugar, insulin resistance, and Type 2 diabetes all place enormous strain on multiple organs. In patients already battling liver fat buildup, the extra metabolic burden speeds up liver damage.
So while hypertension topped the list, diabetes remains a serious and dangerous co-condition.

The third key risk factor is low HDL cholesterol—often called the “good" cholesterol. Patients with MASLD and low HDL had about a 15% higher risk of death than those with healthier HDL levels.
