Think Fatty Liver Is Harmless? Study Reveals How Common Disease Can Turn Deadly
A new study from Keck Medicine of USC has uncovered which risk factors are most dangerous for people with a very common liver condition—Metabolic Dysfunction–Associated Steatotic Liver Disease (MASLD), formerly known as non-alcoholic fatty liver disease (NAFLD). The findings challenge long-held assumptions about what truly raises the risk of death for patients.
What Is MASLD?
MASLD occurs when excess fat builds up in the liver, impairing its function over time. While it may sound harmless at first, the condition can progress to inflammation, scarring, or even liver failure.
What makes MASLD especially tricky is that it often co-exists with other metabolic conditions such as obesity, diabetes, hypertension, and cholesterol issues. Not all of these factors carry equal weight, however, and the new study sheds light on which ones matter most for survival.
High Blood Pressure: The Silent Killer
The most surprising finding? High blood pressure (hypertension) turned out to be the deadliest partner to MASLD. Patients with both MASLD and hypertension had about a 40% higher risk of death compared to those without high blood pressure.
This is significant because both patients and doctors have long focused more on blood sugar or obesity when treating fatty liver disease. The study suggests that blood pressure management may be even more urgent than previously thought.
Hypertension damages blood vessels, strains the heart, and weakens organs—including the liver. For someone with MASLD, that extra stress can push the liver over the edge.
Diabetes Still A Major Threat
The second big risk factor is diabetes or prediabetes. The study found that MASLD patients with impaired glucose regulation had about a 25% higher risk of death.
Elevated blood sugar, insulin resistance, and Type 2 diabetes all place enormous strain on multiple organs. In patients already battling liver fat buildup, the extra metabolic burden speeds up liver damage.
So while hypertension topped the list, diabetes remains a serious and dangerous co-condition.
Low HDL Cholesterol: The Subtle Risk
The third key risk factor is low HDL cholesterol—often called the “good" cholesterol. Patients with MASLD and low HDL had about a 15% higher risk of death than those with healthier HDL levels.
HDL helps clear cholesterol from the bloodstream, reduces inflammation, and offers protective benefits. Without it, the risks tied to fatty liver disease multiply.
Obesity, one of the most common conditions linked with MASLD, also raises risks—but in a more gradual, dose-dependent way. The study found that the higher the Body Mass Index (BMI), the greater the risk of death. In other words, not all obesity carries the same danger—it depends on how high the BMI climbs.
Perhaps the most alarming finding was how risks stack up. Each additional metabolic risk factor increased the risk of death by about 15%. That means a patient with MASLD, hypertension, diabetes, and low HDL cholesterol is at far greater risk than someone with only one of those conditions.
What Patients Can Do
The key takeaways from the study for the MASLD patients are as follows
Check and control your blood pressure. Lifestyle changes—like reducing salt, losing weight, and exercising—can help, along with prescribed medications.
Manage or prevent diabetes. Monitor blood sugar, stay active, and follow medical advice on diet and medication.
Boost your HDL cholesterol. Exercise regularly, eat healthy fats, and avoid smoking.
Aim for a healthy weight. Even modest weight loss can ease liver strain.
Address all risk factors. Each one matters, and together they multiply risk.
Work with your doctor. A comprehensive plan should target blood pressure, blood sugar, cholesterol, and weight—not just one issue.
Treat lifestyle as medicine. Diet, sleep, stress management, and activity all play crucial roles in liver and metabolic health.
The Keck Medicine study underscores a powerful truth: fatty liver disease is not harmless. Left unchecked, MASLD combined with other metabolic conditions can significantly shorten life expectancy.
High blood pressure, diabetes, and low HDL cholesterol stand out as the most dangerous risk factors—but obesity and the combination of multiple conditions raise the stakes even higher.

The good news? With early detection, lifestyle changes, and proper medical care, patients can manage these risks and protect their liver health.
If you have MASLD, don't just watch your blood sugar or weight—watch your blood pressure, cholesterol, and overall metabolic health. It could save your life.
