Obesity's Skin Impact: Understanding Dark Patches, Infections, And Psoriasis As Key Indicators

When we talk about obesity, the discussion is nearly always drawn towards a number, blood pressure, cholesterol, blood sugar, or the scale. Being a medical professional, Dr Vikas Singhal, Associate Director, GI Surgery, GI Oncology and Bariatric Surgery, Gastrosciences, Medanta, Gurugram, frequently encounters patients who care deeply about their internal health yet overlook the biggest organ that is often “screaming" for attention, the skin.

Dr Singhal explains that the skin reflects internal metabolic health. In his practice, he has observed that one of the earliest signs that excess weight is taking a toll on the body appears on the skin. According to Dr. Singhal, interpreting these early warning signs can be the key to timely intervention and an improved quality of life.

Acanthosis Nigricans: A Warning in Velvet

Acanthosis Nigricans is one of the most common but poorly understood skin signs that Dr Singhal encounters. Many patients come to him believing they have an unresponsive tan or a hygiene issue on their neck, armpits, or groin.

It appears as dark, soft, thickened patches of skin and it is not caused by dirt or sun exposure. Dr Singhal explains that it is a direct result of insulin resistance. When the body struggles to use insulin due to excess weight, insulin levels rise, triggering rapid skin cell growth that creates the characteristic velvety texture. It is essentially a red flag for pre-diabetes or Type 2 diabetes even before a blood test confirms it.

The Battle in the Folds: Infections and Skin Tags

Friction caused by skin folds significantly worsens obesity-related skin problems. Dr Singhal frequently sees skin tags (acrochordons) tiny, soft hanging pieces of skin especially around the neck and eyelids of overweight individuals. While harmless, they are often linked to underlying metabolic syndrome.

More concerning, however, are the infections. Obesity creates skin folds that trap moisture and heat, an environment that compromises the skin barrier and leads to intertrigo, a friction-induced rash that easily becomes infected with bacteria or fungi such as Candida. Dr. Singhal emphasizes that these painful, itchy, and sometimes odorous infections aren't just medical , they affect one's emotional and social well-being. And they require medical management, not just more powder or soap.

Inflammatory Cycle: Psoriasis

One of the most significant observations in this field, Dr Singhal notes, is the strong link between psoriasis and obesity. Psoriasis is an autoimmune condition, but excess body weight can worsen it dramatically.

Fat tissue is biologically active and releases inflammatory cytokines. Because psoriasis itself is an inflammatory disease, excess weight adds fuel to the fire triggering flare-ups or making existing psoriasis harder to treat. This creates a vicious cycle: inflammation makes weight loss harder, and excess weight worsens inflammation.

When you notice dark patches, rashes, or psoriasis that doesn't respond to treatment, Dr Singhal urges you not to ignore or hide them with cosmetics. According to him, treating the skin often begins with addressing weight.

Dr Singhal has seen that sustainable weight management can lead to regression of Acanthosis Nigricans, halt the development of new skin tags, and make psoriasis significantly more responsive to treatment. Your body is speaking and, as Dr Singhal stresses, you need to listen.
