Not Just A Teenage Problem: Scoliosis And Daily Pain Can Happen To Adults
When most people hear the word “scoliosis," they picture a teenager with uneven shoulders or an unusual posture. What many do not realise is that scoliosis can begin, or become significant, much later in life as well. In adults, it is usually not a carryover from childhood but a condition shaped by time, wear, and imbalance.
Adult scoliosis is most commonly degenerative scoliosis. The spinal discs lose hydration, facet joints wear unevenly, and supporting ligaments thicken and stiffen. “When this degeneration occurs asymmetrically, the spine gradually begins to curve sideways. That sideways bend is what we call scoliosis," explains Dr Amrithlal A Mascarenhas, Consultant – Spine Surgery, Manipal Hospital, Millers Road.
Unlike adolescent scoliosis, which is often painless, pain is one of the earliest symptoms of adult scoliosis. Patients typically come to the clinic because of persistent back pain, stiffness, or fatigue while standing or walking.
Dr Amrithlal A Mascarenhas says, “Degenerative scoliosis usually appears after the age of 40 and most commonly affects the lower back. Mild curves, measuring 10 degrees or less, may cause little more than occasional discomfort. Moderate curves, if left untreated, can cause increasing pain, muscle strain, and reduced mobility." When curvature progresses severely, there is a risk of nerve compression that may result in leg pain, numbness, weakness, and difficulty walking.
A simple X-ray often suffices for the detection of the degree of curvature, bone quality, joint wear, and spinal alignment. While joint degeneration and spinal rotation can be assessed through CT scans, MRI scans identify nerve compression, spinal canal narrowing, and other hidden causes of pain.
Treatment depends on the severity. Dr Amrithlal A Mascarenhas says, “Mild to moderate scoliosis with manageable pain is best treated non-surgically through physiotherapy, core strengthening, posture training, weight management, and overall conditioning."
However, adults with severe cases of scoliosis, with notable deformity or disabling symptoms, require surgery. Age is usually not a barrier to scoliosis surgery if overall fitness, medical conditions, and recovery potential are favourable.
The most important message is this: adult scoliosis is not just a curve on an X-ray. It is a lived experience of pain, limitation, and adaptation that can be effectively managed with timely detection and appropriate medical care.
