India’s Free HPV Vaccination Rollout Marks A Defining Moment In Cervical Cancer Prevention

India is entering a new phase in women’s healthcare with the Government of India’s decision to introduce free HPV vaccination for adolescent girls through its public health system. The move is being seen as a landmark preventive intervention aimed at reducing the country’s cervical cancer burden, which continues to affect thousands of women each year despite being largely preventable.

Public health experts say the significance of the programme lies not just in the vaccine itself, but in its accessibility.

“The Government of India’s decision to roll out free HPV vaccination is a great step towards strengthening preventive healthcare for adolescent girls and reducing the long-term burden of cervical cancer in the country," says Dr Rita Kalra, Rotary District 3080 Coordinator – HPV Vaccination.

Human papillomavirus (HPV), a common viral infection transmitted through skin-to-skin contact, is responsible for the vast majority of cervical cancer cases. Vaccination during early adolescence ideally between the ages of 9 and 14 offers the strongest protection, as it helps the immune system build defence before any exposure to the virus.

India carries a particularly heavy share of the global cervical cancer burden. According to the World Health Organization, more than 3 lakh women die from cervical cancer globally every year, and a significant proportion of these deaths occur in low- and middle-income countries, including India.

“India alone accounts for nearly one-fifth of the global cervical cancer burden, underscoring the urgent need for large-scale preventive interventions," notes Dr Kalra.

Although HPV vaccines have been available in India for over a decade, they were largely confined to private healthcare, limiting access for many families due to cost and lack of awareness.

“Although HPV vaccines have been available in India through private healthcare channels for years, access remained limited due to affordability, leaving a large proportion of eligible girls unprotected during the most critical age window of 9–14 years," she explains.

By offering the vaccine free of cost, the government’s programme aims to remove this financial barrier and ensure that protection is available more equitably.

“By making the vaccine available free of cost, the programme addresses an issue of health equity and ensures that protection against cervical cancer is no longer dependent on socioeconomic status," says Dr Kalra.

The shift also reflects a broader evolution in India’s healthcare priorities from focusing primarily on treating advanced disease to preventing it in the first place.

“This move shifts the focus of women’s healthcare from treatment to prevention, an essential transition for a country where cervical cancer continues to remain one of the most preventable yet prevalent cancers among women," she adds.

Beyond its medical impact, the initiative may also help open up much-needed conversations around adolescent health and cancer prevention, topics that are often overlooked due to social stigma or lack of awareness.

“Beyond vaccination, the initiative also provides an opportunity to normalise conversations around adolescent health, reproductive health awareness, and cancer prevention. Building awareness and parental confidence will be key to successful implementation, as hesitation often stems from misinformation or limited understanding of the vaccine’s safety and long-term benefits," says Dr Kalra.

Health experts emphasise that vaccination works best when supported by community trust, awareness campaigns, and sustained public engagement, areas where India has previously demonstrated success.

“Rotary’s long-standing experience in mobilising communities during India’s polio eradication campaign demonstrates that large-scale public health challenges can be successfully overcome through awareness, trust-building, and grassroots engagement," she points out.

Rotary International has been among the organisations advocating for wider HPV vaccination coverage, and Dr Kalra says the nationwide rollout marks an important breakthrough.

“Having advocated HPV vaccination for several years, Rotary welcomes this turning point and remains committed to supporting efforts that expand awareness and acceptance. With collective action and community participation, cervical cancer prevention has the potential to become India’s next major public health success story," she says.

Experts believe that if implemented effectively and accepted widely, the programme could significantly reduce cervical cancer cases over the coming decades, protecting millions of young girls today and reshaping the future of women’s health in India.
