How Extreme Weather Disrupts Menstrual Cycles Through Hormonal Stress Pathways
As climate patterns become increasingly unpredictable, many women are noticing something unexpected: their menstrual cycles shifting with the weather. Extreme heat waves or unusually cold spells don't just affect comfort, they can influence hormonal balance, ovulation, bleeding patterns, PMS symptoms, and overall reproductive health. Two leading specialists, Dr Tripti Raheja, Director, Obstetrics & Gynaecology, CK Birla Hospital (R), Delhi, and Dr Swati Sinha, Senior Consultant, Obstetrics & Gynecology, Rosewalk by Rainbow Hospitals, explain how temperature extremes interact with the body's stress systems in ways that can directly alter the menstrual cycle.
Heat: When High Temperatures Trigger Hormonal Stress
According to Tripti Raheja, extreme heat places significant physical stress on the body, triggering the release of cortisol, the primary stress hormone. “High temperatures lead to the release of cortisol, a stress hormone that suppresses the hypothalamo-pituitary-ovarian (HPO) axis. When the HPO axis is suppressed, ovulation may not happen, which can cause delayed or missed periods," she explains.
This suppression of ovulation can manifest as:
Delayed or skipped periods
Shorter or longer cycles
Changes in PMS intensity
Irregular spotting
Expanding on this mechanism, Dr Sinha adds that prolonged exposure to heat elevates the body's core temperature and activates the stress response even further. “High temperatures elevate cortisol, the body's primary stress hormone, which can interfere with normal reproductive hormone signalling. When this signalling is disrupted, ovulation may be delayed or may not occur at all," she explains.
Heat also contributes to dehydration, which affects cervical mucus quality, follicle development, and overall reproductive function. Women living in regions experiencing frequent heatwaves often notice irregular cycles that align with these physiological stressors.
Cold Weather: Subtle but Significant Hormonal Disturbances
While heat overstimulates cortisol, extreme cold influences menstrual health through different hormonal and physiological routes.
Dr Raheja notes that “cold weather can impact thyroid function and melatonin levels, both of which are important for regulating reproductive hormones." When thyroid hormones fluctuate, menstrual cycles can become unpredictable, especially for women with pre-existing thyroid disorders, PCOS, or those approaching menopause.
Cold can cause:
Delayed cycles
Heavier bleeding
Stronger cramps
Increased PMS-related mood changes
Dr Sinha further highlights that reduced sunlight in winter lowers levels of vitamin D, serotonin and melatonin, which indirectly influence the HPO axis. “Cold weather constricts blood vessels and reduces blood flow, which can intensify cramps and sometimes cause heavier menstrual bleeding. These hormonal shifts may lead to cycle delays, mood-related PMS symptoms or altered bleeding patterns," she explains.
Why the Body Reacts This Way: Survival Over Reproduction
Both experts emphasize that in extreme weather whether heat or cold the body shifts its priorities. Survival takes precedence over reproduction. Energy and hormonal resources are diverted toward keeping vital organs stable, which temporarily suppresses reproductive processes such as ovulation.
In both temperature extremes:
The HPO axis becomes dysregulated
Hormone levels fluctuate
The menstrual cycle becomes irregular
As Dr. Sinha puts it, “In both extreme heat and cold, the body prioritises survival over reproduction, leading to hormonal changes that cause menstrual irregularities."
How Women Can Protect Themselves
While you can't control the weather, you can reduce the physiological stress it creates. Both experts recommend:
Limiting prolonged exposure to excessive heat or intense cold
Staying well-hydrated, especially during heatwaves
Maintaining regular exercise to stabilise hormonal pathways
Improving sleep routines, which directly support the HPO axis
Monitoring menstrual patterns, especially if changes persist
Dr Raheja advises that if irregularities continue for more than two consecutive cycles, it's important to consider underlying medical causes beyond just weather-related stress and consult a healthcare professional.
