How Anaya Bangar Transitioned From Man To Woman: Doctor Explains Surgery
Gender-affirming surgery has once again come into public discussion after Anaya Bangar, child of former cricket coach Sanjay Bangar, shared their transition journey. Anaya had earlier undergone procedures as part of transitioning from male to female and will now undergo further gender-affirming surgery in Thailand in March.
To understand the medical aspects of such procedures, News18 spoke with Dr Chirayu Parwal, a plastic surgeon at Aakash Healthcare who specialises in gender-affirming surgeries. He explained the process, purpose and medical considerations involved in transitioning from male to female or female to male.
Gender-affirming surgery refers to medical procedures that help align a person's physical body with their gender identity. Dr Chirayu Parwal explained that the term itself indicates confirmation of gender. These surgeries involve altering physical features and body structures to reflect the gender a person identifies with.
For someone transitioning from male to female, this may include developing breasts and creating female genitalia. Various additional procedures can also be done to reshape physical appearance, including changes to facial features, body contours and other characteristics. Hormonal therapy is often used alongside surgical procedures. The aim is not only cosmetic change, but to bring harmony between a person's identity and their body.
Dr Chirayu Parwal said that this is a complex and sensitive subject that must be understood medically. Some individuals feel that their thoughts, emotions and sense of identity do not match the biological sex they were assigned at birth. In such situations, their internal identity and physical body may feel misaligned.
Before considering surgery, doctors assess hormonal and psychological factors. If no underlying condition is found and the individual continues to feel distress due to this mismatch, gender-affirming procedures may be considered. Ultimately, the decision depends on the individual's personal feelings, mental readiness and choice.
According to Dr Chirayu Parwal, not everyone requires surgery. Some people feel more comfortable in their identity after hormonal therapy alone and live their lives without undergoing surgical procedures. The need for surgery varies from person to person and depends on their feelings, physical characteristics and personal wishes.
Dr Chirayu Parwal explained that gender-affirming treatment can include two major categories, top surgery and bottom surgery. Top surgery for those transitioning from male to female involves breast augmentation. This can be done using implants or fat grafting from other parts of the body. For those transitioning from female to male, chest reconstruction or breast removal is done.
Bottom surgery may involve procedures such as vaginoplasty to create female genitalia. In addition to these, facial feminisation procedures may be performed to soften facial features by altering the jawline, nose, chin, eyebrows and other areas. Multiple procedures may be done over time depending on the person's goals.
Anaya Bangar has already undergone top surgery and is now expected to undergo vaginoplasty, which involves creating female genitalia. Dr Chirayu Parwal explained that this is a complex procedure carried out in stages. It may include removal of the testicles and reconstructing tissue to form a vagina using existing genital structures.
The goal is to create anatomy that resembles female genitalia while preserving sensation as much as possible. Advanced surgical techniques are used to shape the vaginal structure and external features.
Dr Parwal said that surgeons aim to preserve nerves responsible for sensation during reconstruction. Hormonal therapy also plays a role in the body's response after surgery. While many people are able to experience sexual pleasure, results can vary from person to person, and no outcome can be guaranteed to be exactly the same as natural anatomy.
Dr Parwal explained that after transitioning from male to female, a person will not have ovaries or a uterus. As a result, they will not be able to produce eggs or conceive a child. However, they may still have a sexual life, depending on their recovery and overall health.
The cost varies by location and hospital. In India, top surgery may cost approximately Rs 2-3 lakh, while vaginoplasty may cost around Rs 3-4 lakh. Costs can differ based on the complexity of the case and medical facilities.
Dr Chirayu Parwal said that vaginoplasty usually takes about 2-4 hours. After the operation, rest and close medical supervision are necessary for proper recovery.
Thailand is known for its advanced facilities and experience in gender-affirming procedures, and people from across the world travel there for treatment. The procedures are often more affordable than in many Western countries, and some individuals prefer going abroad for privacy and discretion.
