In the UK, the move towards “pharmacy-led" weight management is designed to alleviate the burden on a stretched NHS. However, the transition of Prescription-Only Medicines (POMs) like semaglutide into a walk-in service model has raised significant ethical eyebrows. Critics argue that the high-street setting can sanitise the seriousness of these drugs, which are potent GLP-1 receptor agonists that fundamentally alter digestive and neurological processes.
The primary health concern remains the lack of longitudinal medical supervision. While pharmacists are highly trained, a walk-in consultation may lack the forensic depth of a GP’s history-taking, particularly regarding rare but severe risks such as pancreatitis, kidney complications, and “stomach paralysis" (gastroparesis). Furthermore, the UK’s Medicines and Healthcare products Regulatory Agency (MHRA) has already flagged a rising tide of counterfeit pens, highlighting that as these drugs move from clinical settings to commercial ones, the supply chain becomes increasingly vulnerable to illicit actors.
