From Cramps To Infections: Early Reproductive Health Red Flags Every Woman Must Know
For generations, women have been taught to power through discomfort. Missed classes, sick days during periods, unusual discharge, bloating, or pelvic heaviness are often brushed aside as ‘just part of being a woman.’ But what gets normalised can quietly turn into something far more serious. Behind these everyday inconveniences may lie infections, hormonal disorders, endometriosis, fibroids, or even precancerous conditions – many of which directly impact fertility and long-term reproductive health.
“Severe period pain that disrupts work or school is often dismissed as something that is part of menstrual cycles and women are expected to tolerate it," said Dr Madhulika Singh, Fertility Specialist at Birla Fertility & IVF, Prayagraj. She noted that dysmenorrhoea affects up to 70–90 per cent of young women, but nearly 10–20 per cent experience pain severe enough to limit daily life – often signalling endometriosis or adenomyosis, both strongly linked to infertility.
Similarly, Dr CP Dadhich, Director of Obstetrics and Gynaecology at CK Birla Hospitals, Jaipur, emphasised that worsening cramps, irregular cycles, or recurrent painkiller use are never routine. “Conditions like endometriosis, fibroids, pelvic inflammatory disease or hormonal imbalances often present this way and can go undiagnosed for years," he explained.
Adding to this, Dr Parnamita Bhattacharya, Gynaecologist at CK Birla Hospitals CMRI, Kolkata, warned that symptoms like abdominal heaviness, unexplained swelling, or persistently heavy bleeding may point to fibroids or uterine growths that are frequently detected late.
Painful periods that steadily intensify are often the first red flag. Dr Singh highlighted that heavy bleeding alongside persistent pain may indicate endometriosis – a condition estimated to affect 10 per cent of women globally and up to half of infertile women. Left untreated, it can damage ovarian reserve and reproductive outcomes.
Dr Dadhich added that pain interfering with daily activity should always trigger evaluation. The body, he suggested, rarely escalates discomfort without cause. Early ultrasounds or hormonal assessments can catch problems long before they affect fertility.
Not all discharge is harmless. While some amount is natural, a strong odour, unusual colour, itching, or burning can signal infection. Dr Singh pointed to bacterial vaginosis as one of the most common culprits worldwide. Untreated, infections can travel upward and lead to pelvic inflammatory disease, which significantly increases the risk of tubal infertility and ectopic pregnancy.
Dr Dadhich echoed this concern, noting that fungal or sexually transmitted infections may also present subtly at first but carry long-term consequences. Shame or hesitation often delays care, allowing manageable issues to become chronic.
Beyond pain and discharge, Dr Bhattacharya drew attention to lesser-discussed symptoms: increasing abdominal girth, a sensation of heaviness, bleeding between cycles, or contact bleeding. These could indicate fibroids, intrauterine pathologies, or even precancerous and cancerous lesions of the uterus or cervix.
Persistent heavy periods after 45, especially with obesity or a family history of cancer, deserve immediate medical attention rather than quiet acceptance.
Encouragingly, early intervention is rarely aggressive. A simple swab test, ultrasound, or short course of medication may resolve the issue quickly. What matters most is timing. Listening to the body, tracking changes, and scheduling regular gynaecological check-ups can prevent years of complications. Reproductive health symptoms are not inconveniences to endure, but they are signals to investigate. Addressed early, they protect not just fertility, but overall well-being and quality of life.
