Joe Biden’s Prostate Cancer Spreads To Bones: What Is Bone Metastasis And How Does It Affect The Body?
Former US President Joe Biden is living with advanced prostate cancer that has spread beyond the prostate and into his bones and further, according to a recent update from his son Hunter Biden. In an interview with the BBC, Hunter described the disease as painful and debilitating. Biden was initially diagnosed with an aggressive form of prostate cancer in May 2025.

A high Gleason score indicates that prostate cancer cells look significantly abnormal under a microscope and are more likely to grow and spread quickly. A Gleason score of 8–10 is considered high-grade prostate cancer and carries a greater risk of metastasis.
Hunter Biden described the disease as “very painful and it’s very debilitating, in many respects." He added that the cancer “has metastasized into his bones and further."
Jill Biden had previously confirmed in a June 2025 interview that the cancer had already spread to the bones, noting that this made it “a whole different story" compared with localized prostate cancer. She also stated that Joe “will live with cancer till the rest of his life." Hunter later shared that his father continues to fight: “As long as he’s alive, I promise you, he is going to continue to fight."
Bone metastasis occurs when cancer cells from the original tumour travel to the bones and begin growing there. In prostate cancer, tumour cells can spread through the bloodstream or lymphatic system and commonly affect the spine, pelvis, hips and ribs.
Bone metastasis does not mean a person has developed a new type of bone cancer. Rather, it means prostate cancer cells have established secondary tumours in the bones. Prostate and breast cancers are among the cancers that most commonly spread to bone. When prostate cancer has spread to distant organs or bones, it is considered advanced or metastatic prostate cancer, also known as Stage IV disease.
Importantly, bone metastasis is not the same as primary bone cancer. The cancer cells in the bone remain prostate cancer cells, even though they are now growing in another part of the body.
Prostate cancer can become metastatic when cancer cells acquire the ability to move beyond the prostate and establish themselves elsewhere in the body. Advanced prostate cancer is more likely to spread to distant organs and tissues, including the bones.
Biden’s Gleason score of 9 indicates that his cancer cells showed features associated with a high risk of aggressive behaviour.
Bone metastasis can interfere with the normal structure and function of bones and may cause several complications. Persistent, deep or aching pain is one of the most common symptoms; it may worsen at night or with movement and can significantly affect sleep and daily activities.
Cancer can weaken bone tissue, increasing the risk of fractures, sometimes after relatively minor injuries or falls. Bone damage can also release calcium into the bloodstream, leading to hypercalcemia, with symptoms such as nausea, excessive thirst, dehydration, constipation, weakness and confusion.
Although metastatic prostate cancer is generally not considered curable, treatment can slow cancer growth, control symptoms and improve quality of life. Options depend on the extent of the cancer, previous therapies and the patient’s overall health. Doctors may use hormone therapy (androgen deprivation therapy), chemotherapy, radiation therapy, targeted treatments and radiopharmaceuticals.
Medicines such as bisphosphonates or denosumab may help reduce bone-related complications. Surgery can sometimes be considered for fractures or areas of severe bone instability. Treatment at this stage focuses on controlling the cancer throughout the body rather than treating only the prostate.
Once prostate cancer has spread to distant parts of the body, it is considered metastatic or advanced prostate cancer. While treatments can control the disease and help manage symptoms, metastatic prostate cancer is generally not considered curable.
Treatment decisions depend on several factors, including the extent of spread, cancer characteristics, overall health and previous treatment. Biden’s treatment has included hormone therapy and radiation therapy, according to previous reports.
When prostate cancer reaches Stage IV with bone involvement, the focus shifts to long-term management. Advances in therapy mean many patients can receive treatments designed to slow disease progression and manage symptoms for extended periods. Early recognition of worsening bone pain, weakness, difficulty walking or neurological symptoms is essential so complications can be evaluated and treated promptly.
