A patient once told me that her blood sugar reading in the morning was higher than the one she had taken after dinner the night before. She hadn’t eaten a single thing.
“How is this even possible?" she asked. “I haven’t had breakfast yet."
That question comes up in my clinic almost every week, and the answer usually has nothing to do with food at all. It has to do with everything that happens between waking up and the first bite of breakfast.

The first thing that quietly works against people is the strong, sugary chai many of us reach for the moment we wake up. On an empty stomach, with no protein or fibre to slow things down, that sugar can enter the bloodstream quickly. For someone whose insulin is already struggling to keep up, the rise in blood sugar may be sharper and last longer than it would later in the day.
Dr Singh explains that this is particularly easy to overlook because many people don’t consider their morning chai to be a meal or even a significant source of sugar.
The second mistake is skipping movement entirely in those first waking hours. Muscles are the body’s biggest consumer of glucose, and a body that goes from bed to sofa to desk without so much as a short walk is leaving that glucose sink switched off, exactly when cortisol, which naturally rises in the morning is already nudging blood sugar upward.
A short walk or some light movement after waking can therefore be a simple way to get the body using glucose rather than remaining sedentary.
The third mistake is what actually breaks the overnight fast. A breakfast built almost entirely around refined flour or sugary cereal, with little protein, can do the opposite of what people expect. It may lead to a rapid rise in blood sugar followed by a drop, potentially leaving a person hungrier and more likely to crave sugar within a couple of hours.
According to Dr Singh, a protein-forward breakfast can be a better option. Eggs, paneer or another balanced breakfast containing adequate protein can help make the morning meal more satisfying and support steadier blood sugar levels.
The fourth mistake is poor sleep the night before. A short or interrupted night doesn’t just leave someone tired; it can also affect insulin sensitivity the next morning. As a result, the same breakfast that was well tolerated after a good night’s sleep may produce a higher blood sugar reading after a poor night’s sleep.
Dr. Singh says sleep is often an overlooked part of blood sugar management, even though it can influence how the body responds to food and stress the following day.
The fifth, and perhaps the one people underestimate the most, is starting the day already stressed, scrolling through messages, checking work notifications or rushing out the door with the body’s stress response fully switched on before the day has even begun.
That stress response can cause the body to release hormones such as cortisol and adrenaline, which can raise blood sugar even when no food has been consumed.
Dr Singh points out that this is one reason why the number on the glucometer isn’t always simply a reflection of what someone ate the previous evening.
None of these mistakes involve the food choices people typically think to question, and that’s exactly why they can go unnoticed for years.
The good news is that none of them necessarily requires a dramatic lifestyle change. A calmer first hour, a short walk, a protein-forward breakfast and a genuine effort to maintain consistent sleep can all contribute to better morning blood sugar management.
My patient’s fasting reading came down within weeks, not because she changed her breakfast alone, but because she changed the hour before it.

Reducing stress in the first hour of the day can also help.
Stress can cause the body to release hormones like cortisol and adrenaline, which can increase blood sugar levels even without food consumption. This is why blood sugar readings are not always solely a reflection of what someone ate. Stress hormones can also impact liver performance.
