Breathlessness Blamed On Pollution: When Lung Cancer Hides Behind Poor Air Quality
As air quality index (AQI) levels increase, breathlessness has become the new normal, often blamed on seasonal smog. In cities where a thick grey canopy has become part of the permanent skyline, a persistent cough or tightness in the chest is rarely viewed as a medical emergency. Instead, it is treated as a routine part of urban life.
People check the AQI, buy an extra air purifier, and assume their bodies are simply reacting to the smog. However, this casual acceptance of breathlessness is creating a dangerous diagnostic trap. By the time someone realises that their “pollution cough" is actually caused by a malignant tumour, the window for effective treatment may already have closed.

While tobacco remains a well-known cause of lung cancer, long-term exposure to polluted air is now classified as a Group 1 carcinogen. People should watch for warning signs before they become severe indicators of lung cancer.
Persistent cough: A cough that does not clear up after three weeks, even during periods of cleaner air or while staying indoors.
Unusual fatigue or unexplained weight loss: Feeling constantly exhausted despite adequate rest, or noticing weight loss without changes in diet or physical activity.
Changes in voice or chest discomfort: Hoarseness that feels deeper than a typical sore throat, or sharp pain when taking a deep breath.
Severe or persistent congestion: Dependence on inhalers or cough syrups that provide little or no relief, suggesting a structural problem rather than simple inflammation.
Lung cancer is increasingly being diagnosed in younger, non-smoking individuals. Because PM2.5 particles are microscopic, they can bypass the body’s natural defence mechanisms and settle deep within lung tissue, causing chronic, low-level damage.
Doctors are seeing a rising number of cases among non-smokers, young professionals, and women living in traffic-heavy areas, many of whom are being diagnosed with aggressive forms of adenocarcinoma.
The risk becomes significantly higher for chronic smokers who are also exposed to polluted air. Tobacco smoke and air pollution act together, accelerating lung injury and increasing the likelihood of cancer.
For people in this high-risk category, low-dose CT (LDCT) screening can help detect lung cancer at an early stage, often before symptoms appear.
When the disease progresses beyond the point where medication alone can help, treatment requires a multidisciplinary approach, including surgery, radiation therapy, and chemotherapy.
In surgical cases, doctors may perform procedures such as a lobectomy, which removes an entire lobe of the lung, or a pneumonectomy, where the entire lung is removed in more severe situations. The goal of these procedures is to remove the primary tumour along with affected lymph nodes, stopping the spread of a disease that may have initially been masked by pollution-related symptoms.
One of the biggest challenges in diagnosing lung cancer is that traditional diagnostic assumptions often lead to delayed detection. A standard chest X-ray can easily miss tiny nodules hidden behind inflammation caused by polluted air.
To overcome these limitations and enable early, accurate diagnosis, several advanced tests play a crucial role:
Chest X-ray: A basic first-line investigation, though limited in detecting small or early-stage lesions.
Low-Dose CT (LDCT) Scan: The most effective screening tool for early detection, especially for high-risk individuals.
Contrast-Enhanced CT (CECT) Chest: Provides detailed information about tumour size, location, and lymph node involvement.
PET-CT Scan: Helps determine whether the cancer has spread and assists in staging and treatment planning.
CT-Guided Lung Biopsy: Confirms the diagnosis by collecting tissue samples from suspicious lung lesions.
Air pollution does not just make breathing difficult, it alters the biological landscape of the lungs, increasing the risk of serious disease. The danger becomes even greater when symptoms are ignored, allowing underlying conditions to remain hidden until it is too late.
