From creating songs and aesthetic images to now assisting in surgery, AI has come a long way. Recently, Rhys Hibbert from England has become the world’s first patient to undergo brain surgery with assistance from AI. Surgeons operating on the 48-year-old father of two successfully removed a tumour that had begun affecting his vision.
Rhys Hibbert underwent the procedure at University College London Hospital, where a bespoke AI system analysed a live video feed during the operation. The technology helped surgeons identify the likely locations of critical blood vessels and nerves hidden from view, allowing them to choose safer areas from which to remove the tumour.
“From a patient safety perspective, I can see absolutely the benefit. There are no road signs inside our head," Hibbert told the BBC.
Until around 18 months ago, Hibbert described himself as fit and healthy. He regularly went for a two-mile walk during his lunch break. During one such walk, however, he suddenly lost consciousness and suffered a seizure.
Doctors subsequently discovered an 11mm non-cancerous tumour on his pituitary gland, a small organ located at the base of the skull.
The pituitary gland produces hormones involved in several important bodily functions, including growth, metabolism and the regulation of blood pressure and temperature. Hibbert’s tumour was particularly concerning because the gland sits close to the carotid arteries, which supply blood to the brain, as well as the optic nerves responsible for vision.
As the tumour grew and began pressing against his optic nerves, Hibbert started losing some of his peripheral vision. He also experienced severe fatigue and dizziness and began tripping over objects.
“For 18 years of married life I’ve always got out of bed and made my wife a cup of coffee," Hibbert said, “and I wasn’t going to let this tumour stop that."
Surgeons did not initially need to operate immediately, but as his symptoms progressed, Hibbert eventually agreed to take part in the experimental procedure.
“If patients are not prepared to join research how can doctors ever learn and how can medicine ever progress?" he said.
The operation involved inserting an endoscope, a thin camera attached to a narrow instrument, through Hibbert’s nose and towards the base of the skull. The tumour was positioned behind layers of bone and tough membrane, making it difficult to identify the safest route for removal.
According to the research team, surgeons can face a 25-50% chance of not completely removing a pituitary tumour and a 0.5-2% risk of damaging a major blood vessel.
The AI system was designed to assist with precisely this challenge. During surgery, it analysed the live camera footage, tracked the surgical instruments and highlighted areas where important blood vessels and nerves were likely to be located.
Researchers trained the technology using hundreds of recordings of pituitary tumour operations. Experts manually identified and marked the relevant anatomical structures in the footage, allowing the system to learn how to recognise them.
Prof Hani Marcus, one of the neurosurgeons involved, said the AI had been trained on more operations than most surgeons would encounter in their careers and could function as an “expert second pair of eyes".
However, the technology did not make surgical decisions independently. The medical team retained complete control throughout the procedure.
AI is expected to impact medical training by integrating AI-powered virtual reality and simulators into MBBS studies, allowing students to practice complex surgeries without harming patients. These tools can also improve diagnostic accuracy. Guidelines for the use of AI in medical education are being developed, and some hospitals are using AI-powered simulation labs for training, where errors lead to the simulated patient's deterioration.
